Change of Status Forms:
Add/Drop/Take a Leave of Absence - click here. Instructions:
- Save/download form to your computer (do not chose the Open option when you click on the file!) Most people save files to their "My Documents" folder
- Double click to open the file in Microsoft Word
- Fill out form, and then do a "Save as..." and name the file Change of Status Form with your name indicated in the file name (e.g. "John Smith - OLMBA001 - Change of Status Form to Drop GBA602.doc")
- Attach file to e-mail and send to e-mail address indicated on the form. Send a copy to your Student Services Specialist.

For Official Transcripts - click here. Instructions:
- Save/download form to your computer.
- Open the file and print out the form. Only paper Transcript Request Forms (with payment) can be sent/mailed in to the Registrar's office to obtain official transcripts.
- U.S. Mail the form to the address indicated on the form.
