Special note: the original deadline for this application was February 1, 2006. If you submit this September 2006 application now, please contact Camille Timpone in the Registrar's Office to determine when your diploma will actually arrive.

Please note the following deadlines for Applications:

  • Students planning to graduate in January, 2007 - October 16, 2006
  • Students planning to graduate in May, 2007 or September, 2007 - February 1, 2007


Remember to download the form to your computer, print the form out and then complete it. Forms (with payment) can only be mailed in via postal mail and not sent electronically. Forms may not be faxed. As with any form, please make a copy of it before you place it in the envelope for mailing!

When filling out the form, please print carefully and indicate the appropriate degree for which you are applying!
