Please note that each change of status form has a specific function. 

• Add or drop a class, use the Change Course Schedule form. Please contact your Program Advisor for form
• Change of name, address or other information, use Change Information form

The forms listed below may only be obtained from your Learner Services Specialist:

• Leave of Absence - Contact your Program Advisor
• Withdraw from program - Contact your Program Advisor
• Change of Cohort - Contact your Program Advisor
