Title  Case Manager
Salary  $14/hr
Description  A full-time position to provide case management and support services to consumers in the Hope for Veterans Program.



Provide on site consumer supervision and clinical support and on-call coverage to staff and consumers for crisis intervention and
emergency services. Case Managers will coordinate and monitor all aspects of service delivery including assessments, evaluations,
recommendations on placements, treatment planning, and case conferences. They assist with referrals for adjunctive services,
family meetings, group work, individual counseling (as needed), consumer advocacy, liaison with other community agencies and
discharges.
Required 
Skills 
BA degree in a mental health discipline or 4 years relevant experience is needed

Mental health/human services experience.

Candidates must also possess a valid driver\'s license with a good driving record.
