Position Information

Title  Full-time Youth Case Manager (NewBridge 70001 Jobs
Description  1. Provides case management and program support in outpatient educational/vocational program assisting youth aged 16-21 who
have left the traditional high school environment.
2. Facilitate or assist with; program orientation, life skills activities, employment skills training, supportive counseling,
academic study and career development.
3. Works with Youth to link them to community services, providing transportation as needed for appointments and service
delivery.
4. Completes monthly follow up activities with Youth who have completed the program, maintaining documentation of communication
and progress.
5. Participates in the enrollment activities for all referrals; answers inquiries about the program, field telephone referrals
and document information, schedules intake appointments, logs all pertinent information, follows up with absentees.
6. Assists with counseling intake assessments and diagnostic activities.
7. Maintains and implements an organized filing system for program materials: inquiry material, enrollment paperwork, benchmark
achievements.
8. Mails or delivers paperwork as needed to administrative or County offices.
9. Prepares statistical reports as assigned, including Grant-in-Aid documentation of client contacts, Employment and Training
Services invoices and copier usage.
10. Collects fees and paperwork for admission, enrollment, and program activity purposes.
11. Other duties as assigned.
Required 
Skills 
Bachelor’s Degree in Social Work, Psychology, Human Services or related field; or Associate’s Degree in Social Service related field
with 2 years relevant work experience
OR
High School Diploma with 5 years of relevant work experience with at-risk or educationally challenged youth.

Computer literate
Good communication skills
Excellent organizational/time management skills
Bilingual Spanish/English a plus
Valid New Jersey Driver’s License
