Title  Case Manager
Salary  $15/Hour
Description  The Warren County Youth Advocate Programs has developed and implemented a Warren County Supported Work and Incentive Program for juveniles involved in the juvenile
justice system. The WSWIP is funded through the Warren County Department of Human Services. The WSWIP provides life skills, employment opportunities and incentives
for boys and girls involved with Warren County Probation between the ages of 15 and 17.
• Conduct initial interviews as well as develop Individualize Service Plans (ISP) for all referred youth
• Provide 2-3 hours weekly of life coaching
• Research, facilitate, build and maintain relationships with current and potential future employers
• Provide weekly supervision and life coaching to youth while employed
• Research, develop and implement incentive trips, college tours and guest speaker
• Prepare, monitor and maintain all client files
• Prepare, monitor and maintain weekly reports
• Facilitate and maintain consistent communication with youth, families, probation, director and all other involved personal
• Participate in all mandatory trainings
• Bachelor’s Degree preferred. Minimum of High School Diploma and equivalent experience with target population as well as in depth knowledge of community resources
